Registration Forms

Welcome to Announcements

Registration and Open House Night: We will have the Kindergarten open house and registration night on Thursday, February 23 from 6:30-7:30pm.

Parent Survey: We are currently conducting a survey of our parents. The surveys were sent home with the February packet. If you are in need of a new one, you can find the link under the PARENTS tab. Thank you for you time and responses.

Update on Fundraisers: We are pleased to announce that due to the success of the Fall Festival, Marathon, and increased scrip usage there will be no need to sell Pizza Pete this school year. Thank you for your support!

Student Incident Report: We have instituted a new system of reporting and documenting student incidents here at St. Felix School for Grades K-6. If your child is spoken to by a staff member for something that they said or did that would merit them going to the office, your child will fill out the “St. Felix School Incident Report”. Your child will write about what they did at school, reflect, and then write about what they can do so that it does not happen again. The student will then sign and date the form.

A photocopy will be made to retain in our records and the original will be sent home. Please read, sign, date, and return the form back to the school. The original will be attached to the photocopy. This system will help us document our discipline procedures and improve communication between students, parents, and staff. If you have any questions, you can contact Mrs. Frost or Mr. Deiss.

Winter Weather Information:

- Please begin to send snow pants, boots, winter jacket, gloves, hat, scarves, etc. The children will need these clothes for recess times.
- If school was delayed, half day Preschool and Pre-Kindergarten would not take place. All day Preschool and Pre-Kindergarten would start at the same time as Grades K-6.
- Tune into local media outlets and/or the school text message alert system to be notified of school being delayed, closed, or closing early.

View Report Card Online: You can view your child's report card online. Simply click on Cornerstone found under the PARENTS heading.

Education City: Please send $6.00 to the school office (checks payable to St. Felix School) and Mr. Deiss will issue your child their 2011-2012 homework pass.

Preschool & Pre-Kindergarten Scrip: We offer scrip tuition assistance for the Preschool and Pre-Kindergarten programs!

Outdoor Dress: We  are in the season of Winter! Please send your child with warm winter clothing to be outside during recess and physical education class. Hats, gloves/mittens, snow pants, scarfs, and snow boots are greatly needed each day.

God's Kids Registration: The God's Kids program is still accepting registrations for AM and PM care programs. Forms available online or from the office.
